A patient with pulmonary embolism and patent foramen ovale

چکیده
Pulmonary embolism (PE) is a significant complication of orthopedic surgery, although it is rare after arthrosocopic surgery. PE is stratified into low, intermediate or high risk, depending on the haemodynamic status of the patient and the presence of markers of right ventricular dysfunction. Both the management and the choice of the appropriate level of care [i.e., intensive care unit (ICU), coronary care unit (CCU), general ward] are dictated by the severity of symptoms and the coexisting risk factors. The hypoxaemia associated with PE is usually due to a low ratio of ventilation and perfusion and consequently it is usually easily corrected with supplemental oxygen. Case Report

Paradoxical embolism complicating massive pulmonary embolus.
Paradoxical embolism is an unusual but well recognized complication of venous thromboembolic disease. This case report of a patient with massive pulmonary embolism, who had a paradoxical embolus through a patent foramen ovale, emphasizes that in these circumstances surgical intervention is indicated.
متن کاملThrombus entrapped by patent foramen ovale in a patient with pulmonary embolism: a case report
Thrombus-in-transit appears to increase the risk of mortality compared to pulmonary embolism alone and can require alteration in therapeutic plan. We present the case of a biatrial thromboembolus caught in transit across a patent foramen ovale diagnosed by intraoperative transesophageal echocardiogram in a 69-year-old female with acute pulmonary embolism and subsequent acute cerebral infarction...
متن کاملPatent foramen ovale is an important predictor of adverse outcome in patients with major pulmonary embolism.
BACKGROUND Right-to-left shunt through a patent foramen ovale is frequently diagnosed by contrast echocardiography and can be particularly prominent in the presence of elevated pressures in the right side of the heart. Its prognostic significance in patients with pulmonary thromboembolism, however, is unknown. METHODS AND RESULTS The present prospective study included 139 consecutive patients...
متن کاملMultiorgan paradoxical embolism consequent to acute pulmonary thromboembolism with patent foramen ovale: a case report
Paradoxical embolism is defined as a systemic arterial embolism requiring the passage of a venous thrombus into the arterial circulatory system through a right-to-left shunt. It is a relatively rare phenomenon, representing about 2% of all cases of arterial embolism. We report a case of a 79-years-old woman admitted to hospital because of dyspnea and lower left limb pain. CT scan revealed multi...
متن کاملHeart Carcinoid Disease with Patent Foramen Ovale Treated by Mini Sternotomy
This case report was an attempt to present the importance of diagnostic echocardiography and closure of a patent foramen ovale in carcinoid heart disease. Herein, we present a case of a 48-year-old woman with a carcinoid metastasis presenting with tricuspid regurgitation, pulmonic stenosis, patent foramen ovale, and borderline left ventricular ejection fraction on an echocardiogram. Surgical in...
متن کاملSurgical Treatment for Thrombus Straddling a Patent Foramen Ovale
We present a case of a 41-year-old female with deep vein thrombosis after abdominal surgery. The patient quickly developed severe pulmonary embolism and stroke representative of paradoxical embolism. Echocardiography showed a thrombus straddling a patent foramen ovale, which was confirmed intraoperatively. An accurate diagnosis and rapid treatment decisions are crucial for preventing patient de...
